Enhancements to Two Standard Document Sets
PRO-245G-E & PRO-245G-S
In response to recent legislative changes to MN State Statute Rule 245G, Procentive is updating two of our standard document sets. If you are using our ‘PRO-245G-‘ documents, you will see these changes take effect with newly created documents beginning July 15, 2019.
PRO-245G-E “Expanded” Document Set Changes
PRO-245G-E1000 Initial Services Plan
- Revised question text related to ‘Identified needs of the client DURING THE TIME BETWEEN DATE OF SERVICE INITIATION and development of the Individual Treatment Plan’.
PRO-245G-E1100 Comprehensive Assessment and Summary
- Text change to narrative asking for a description of the circumstances ON THE DAY OF service initiation.
- Added question regarding medical concerns: Do these concerns need to be referred to an appropriate health care professional?
- Revised question text to: HOW HAS USE AFFECTED YOUR ability to function appropriately in an educational setting.
- Revised question text to: HOW HAS USE AFFECTED YOUR ability to function appropriately in a work setting.
- Revised text to ‘# of Days (in last 30)’ for clarification in Substance Use history tables.
- Added Amount, Frequency, Duration and Leisure Activities fields to Substance Use history tables.
- Combined narrative area for entry of ‘Risk taking behaviors, including behavior that puts the client at risk of blood-borne or sexually transmitted diseases:’ Previously this was two separate areas.
- Revised ‘Client Choices/Preferences’ header
- Revised question text: ‘Does the client recognize NEEDS related to substance use and is willing to follow treatment recommendations?’
- Added question: ‘Does the client desire family involvement in the treatment program?’
- Added definition of ‘Problems’ within each Dimension. Entered problems will dataflow to the Treatment Plan.
- Added Edit List linked to Summary and Recommendations.
- Added checkboxes for verification of client Eligibility, Income, Residency and Insurance.
PRO-245G-E1101 Comprehensive Assessment Summary
- Added definition of ‘Problems’ within each Dimension. If entered previously on the PRO-245G-E1100 Comp Ass & Summary, will dataflow into these areas. Entered problems will dataflow to the Treatment Plan.
PRO-245G-E1200 Treatment Plan
- Added dataflow of Problems from ANY.
- Added dataflow of ‘Client indicated desire for family involvement in the treatment program’ from PRO-245G-E1100 and a new narrative for entry of ‘How will family or others be involved in the client’s treatment plan:’
PRO-245G-E1300 (RENAMED) Documentation of Treatment Services / Treatment Plan Review (prev Progress Note and Treatment Plan Review)
- Added area for identification of Treatment Service Review Period Start Date and End Date.
- Added checkboxes for ‘Denies’ to Care Coordination Activities, Medical Appointments Attended, Other Appointments Attended and Doc of Participation of Others areas.
- Added radio button series to document whether Client’s IAPP has been reviewed and evaluated.
- Added narrative for ‘Reason for Risk Rating’ in each dimension.
PRO-245G-E1400 (RENAMED) Service Discharge Summary (prev Discharge Summary)
- Added dataflow of clients issues, strengths and needs from PRO-245G-E1100 Comp Assessment
